and beyond to enhance the quality of life for staff, patients, and/or families.

• Complete the Nomination Form 
• ONLY submit the Nomination Form; no additional documents will be considered.
• Previous award recipients are not eligible for nomination.
• Following the August 4th deadline, nominations will be vetted and then sent to an out-of-state judge for review/selection. One award recipient will be selected for each category.
• You will be notified of results by August 29, 2025.
• Recipients will be recognized and receive their award on September 22nd, during the 2025 Hospice and Palliative Care Conference in Benton, AR.
